The Implant Post vs. The Crown

It is important to distinguish between the titanium implant post (the part in your bone) and the crown (the visible tooth above the gum). The implant post, once successfully osseointegrated, can last a lifetime with proper care — 20–30 years is typical, and many patients who received implants in the 1980s still have functioning implants today. The ceramic or porcelain crown, however, is subject to normal wear and may need replacing after 10–15 years.

What Causes Dental Implants to Fail?

  • Peri-implantitis: gum disease around the implant — the single most common cause of long-term implant failure. Regular hygienist appointments are essential.
  • Smoking: reduces blood flow to bone and gum tissue; smokers have failure rates roughly double those of non-smokers.
  • Poor oral hygiene: bacteria build-up around the implant leads to bone loss and eventual failure.
  • Bruxism (teeth grinding): uncontrolled grinding places excessive force on implants; a night guard is recommended.
  • Medical conditions: uncontrolled diabetes, certain medications (e.g. bisphosphonates) and autoimmune conditions increase risk.
  • Early loading: putting load on the implant before osseointegration is complete.

How to Make Your Implants Last

  1. Brush twice daily with a soft-bristle brush, paying particular attention to the gum line around the implant.
  2. Use interdental brushes or floss daily — bacteria between teeth spread to implant sites.
  3. Attend your dentist and hygienist every 6 months for professional cleaning and implant health checks.
  4. Stop smoking — or at minimum, do not smoke during the first 3–6 months post-surgery.
  5. Wear a night guard if you grind your teeth.
  6. Avoid biting very hard objects (ice, nutshells) which can fracture the crown.
